Community Intervention services are designed to help individuals overcome barriers to community integration and independent living. Provided on an individual basis, these services address challenges that may otherwise lead to hospitalization or more restrictive living environments. Community Intervention may include:

  • Resolving housing issues or landlord conflicts

  • Assisting with access to social services and community resources

  • Addressing employment or education-related concerns

  • Facilitating communication with agencies, institutions, or family members

  • Preventing hospitalization or higher levels of care

  • Supporting crisis resolution and stability planning

  • Promoting successful community reintegration

By fostering strong partnerships and advocating for client needs, we help individuals build lasting connections and thrive in their communities.

Transition to Community Living

Transition to Community Living (TCL) services assist individuals transitioning from a higher level of mental health care back into the community. As a short-term, goal-focused service, TCL bridges the gap between discharge and independent living by coordinating care, promoting stability, and carrying out the discharge plan developed by the higher-level care provider. These services complement, but do not duplicate, the facility’s discharge planning efforts.

TCL services are provided when:

The individual is being discharged from a higher level of care such as:

  • Regional treatment centers

  • Community hospitals

  • Institutes for Mental Disease (IMDs)

  • Intensive residential treatment programs

  • Board and care facilities

  • Skilled nursing homes

  • ACT (Assertive Community Treatment) programs

Transition to Community Living services cannot overlap with other ARMHS services and is only available while the individual is receiving higher-level care.

Medication Education

Our Medication Education program empowers individuals with the knowledge needed to make informed decisions about their mental health treatment. Delivered by qualified professionals—this service focuses on helping clients understand their mental health conditions and the medications prescribed to manage them. Medication education complements, but does not replace, medication management services.

  • Understanding mental illness and related symptoms

  • How medications help manage symptoms of mental illness

  • Common and potential side effects of prescribed medications

  • The importance of medication adherence and consistency

  • How to communicate concerns or changes with your care provider

Services are customized to each client’s treatment goals and needs, and may include guidance for clients, families, or significant others on properly managing and maintaining the client’s prescribed medication regimen.
