Transition to Community Living
Transition to Community Living (TCL) services assist individuals transitioning from a higher level of mental health care back into the community. As a short-term, goal-focused service, TCL bridges the gap between discharge and independent living by coordinating care, promoting stability, and carrying out the discharge plan developed by the higher-level care provider. These services complement, but do not duplicate, the facility’s discharge planning efforts.
TCL services are provided when:
The individual is being discharged from a higher level of care such as:
Regional treatment centers
Community hospitals
Institutes for Mental Disease (IMDs)
Intensive residential treatment programs
Board and care facilities
Skilled nursing homes
ACT (Assertive Community Treatment) programs
Transition to Community Living services cannot overlap with other ARMHS services and is only available while the individual is receiving higher-level care.
